New clients will receive Health History and clinic policies/cancellation policy forms after you book. Please fill these out a day before your appointment so I can review your health history before you arrive. All clients will receive a prescreen for COVID-19 symptoms with your appointment reminder and completing it before your appointment is a requirement of the Ministry of Health.
