Please fill these out prior to your appointment. Every massage appointment includes an intake and home care. Direct Billing is available for most insurance plans. Please let me know if you would like to set this up and I will email you the forms to fill out prior to your appointment. Otherwise, payment is due in full, immediately after your appointment. Rapid NFR Treatments: Please wear loose, comfortable clothing. Treatments are done without lotion and over your clothing. As of March 1 2024: Payment and Cancellation policy will be sent out for a signature prior to your appointment. Please note: We have pets. They are not allowed in the treatment room, and you will not have access to them, but they do live here.
