If you are booking a first time appointment since our reopening in June, you will be emailed a digital COVID-19 Prescreening document. Please complete this document as soon as possible as it is a mandatory protocol for receiving treatment. It is an important part of keeping everyone and ourselves safe and we greatly appreciate everyone's efforts.
Please bear in mind, failure to complete the document 24 hours before your appointment may result in a cancellation for everyone's safety.
If you have booked a first appointment and have not received any digital paperwork, especially the COVID-19 Prescreening form, please email or call our office and we will have one resent.
For returning patients, we are also taking this time to update patient health histories. It is a mandatory requirement by our college to update this paperwork annually. Our new note-taking and filing system is completely digital, so your new health history will be a digital file that is filled out in a web browser and immediately sent back to us once completed. If you haven't filled out a digital health history since our return in June, please get in touch with us by email or phone and we will have one sent to you.
