Please check your email for health release forms to fill out prior to your appointment if you are a new client or haven’t filled out a covid release form yet. If you do not receive them please contact me.

A valid credit card is required to book your appointment. You will NOT be charged when booking your appointment. If you no call/no show for your appointment then your card WILL be charged for 50% of the missed appointment. Please understand that this is to reserve your appointment for my time and yours. If you’re not going to make it please call.
