Setting up an Additional Form

Practitioners may send additional forms to Patients to ensure they update their health history or provide relevant information for their visits.

1. Go to a Patient's profile.
2. Click on the Forms tab in the sidebar.
3. Click on Add Form button.
4. Select the Form and the Agreements (if any).
5. Choose your method of delivery: Email or Complete Now.
